Mission Neighborhood Health Center (MNHC), a non-profit, multi-site Federally Qualified Health Center (FQHC) located in the heart of San Francisco’s Mission and Excelsior districts. MNHC offers comprehensive primary care services, including adult medicine, pediatrics, family planning, OB/GYN, HIV, and homeless services to an underserved, largely immigrant Latino and Spanish-speaking patient population. We are currently looking for qualified candidates for our Lead Physician role at the Excelsior Adult Medicine clinic. This position reports to the Chief Medical Officer and is a Regular, Full Time, Exempt role with a starting annual salary range of $241,500 to $267,225 with full benefits.  This is a unique opportunity to join a collaborative and supportive team of dedicated providers, enjoying a high level of job satisfaction.

Essential Functions/Responsibilities


Clinical Duties
  • Serve as the primary care provider for adult and elderly patients.
  • Provide comprehensive, ambulatory, preventative medical care in accordance with national standards and guidelines, including those set by the AAP and DHHS.
  • Diagnose and treat acute and chronic medical conditions with evidence-based medicine.
  • Oversee the continuity of care for patients, including managing transitions between inpatient and outpatient settings.
  • Conduct regular health screenings, chronic disease management, and patient education, focusing on prevention and early intervention.
  • Perform routine office procedures such as joint injections, skin biopsies, and wound care management as needed.
  • Provide telehealth services and telephone consultations to patients as appropriate.
  • Other duties as assigned.
Administrative Duties
  • Lead the interdisciplinary medical team to improve clinical and operational metrics (e.g., immunization rates, cycle time).
  • Oversee and regularly review clinical workflows and protocols to ensure quality outcomes and efficient service delivery.
  • Support process improvement initiatives, contributing to MNHC’s recognition as a Patient-Centered Medical Home (PCMH).
  • Collaborate with Clinic Managers, Medical Assistants, and other staff to foster a high-functioning, team-oriented environment.
  • Coordinate with community organizations, the Department of Public Health, and other partners to improve patient care delivery.
  • Lead quality improvement initiatives and implement practice changes as needed in collaboration with other leaders.
  • Mentor and coach providers, ensuring adherence to clinical guidelines and addressing performance issues when necessary.
  • Other duties as assigned.

Qualifications
Education
  • Valid California medical license and current DEA license required.
Experience
  • Board certified or eligible in Family Medicine or Internal Medicine.
Technical Skills
  • Proficiency in Microsoft Office (Outlook, Word, Excel, etc.) and experience with Practice Management Systems and Electronic Health Records (EHR) such as Epic.
Tools and Equipment
  • General office equipment (fax, copier, printer, phone, computer).
Confidentiality
  • Must uphold the highest standards of confidentiality in handling patient records and sensitive information.
Language
  • Bilingual in Spanish and English required.
