Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV, and homeless services. We are dedicated to clinical excellence and health equity through the provision of culturally and linguistically competent services to all our patients, including a commitment to the underserved within the Latino and/or Spanish-speaking communities.

Mission Neighborhood Resource Center (MNRC) is focused on the unhoused population of the Mission District and features a two-exam-room medical clinic offering primary care and urgent care services, as well as substance use counselling and mental health counselling. This clinic is collocated with additional services at the MNRC Drop-in center, which include case management, support groups, shower facilities, lockers, food support, and referrals for lodging and housing.

We are currently looking for qualified candidates for our Nurse Practitioner or Physician Assistant, Part Time or Full Time role. This position reports to the Lead Physician for Special Populations with an annual salary range of $130,400 to $143,500.  

Primary Objective:
The Primary Care NP or PA at Mission Neighborhood Resource Center (MNRC) will have the rewarding responsibility of providing medical services primarily to patients from the homeless population of the Mission District. Upholding our mission of delivering compassionate, comprehensive, and competent care, the provider will actively support team-based care, collaborating closely with medical assistants, nurse, behavioral health providers, case managers, and other staff to advance the clinic's objective of clinical excellence as a patient-centered medical home.

The position available in MNRC is part-time (20 hours/week, i.e. five 4-hour shifts), but it is possible to create a full-time position by combining the MNRC shifts with shifts in other MNHC clinics.

Essential Functions & Responsibilities

  • Provide ambulatory acute, chronic, and preventative medical services to patients in alignment with Community National Standards of Practice and DHHS guidelines.
  • Collaborate with the care team to deliver patient education on wellness, prevention, and early detection, offering materials and resources to patients and their families.
  • Care for homeless patients with attention to assessing and addressing social determinants of health
  • Care for transgender and gender diverse patients, including managing gender-affirming hormones.
  • Care for patients with serious psychiatric disease, including managing antipsychotic medications.
  • Care for patients with substance use disorders, including providing Medication-Assisted Treatment (MAT)
  • Maintain accurate documentation of all services and communications in patient medical records in a timely manner to ensure continuity of care.
  • Monitor laboratory results and patient communications within the electronic health record promptly and effectively.
  • Consult with colleagues, clinical chiefs, and the medical director as necessary to address patient care, assessment, and education concerns.
  • Engage in quality improvement initiatives, management activities, continuing education, peer review, and other patient care programs mandated by regulatory, grant, or clinic requirements.
  • Responsibly order studies, tests, and ancillary services as needed to support patient care.
  • Collaborate with specialists and hospitalists/residents regarding outpatient referrals and patient care plans, adhering to Health Center protocols, and providing continuity of care in transitions between inpatient and outpatient care
  • Complete billing encounters promptly.
  • Perform any other duties as assigned by supervisor.


Qualifications 

  • Current RN/NP or PA License and Furnishing Number issued by the California Board of Registered Nursing.
  • Master's degree in Nursing and/or Doctorate in Nursing Practice, or PA degree
  • Bilingual proficiency in English and Spanish is required.
  • National certification from the American Academy of Nurse Practitioners, American Nurses Credentialing Center, or other recognized national NP or PA certifying body.
  • Preferred minimum of one year of experience as a Nurse Practitioner.
  • Experience with Practice Management Systems and Electronic Health Record Systems is preferred
  • Knowledge of and sensitivity to working with diverse cultures/communities.
  • Ability to maintain the highest level of confidentiality when handling patient records and personal information.
  • As a condition of employment, all candidates are required to meet specific immunization standards. Documentation of current immunizations, including Varicella, TDAP, COVID-19, MMR, and Hepatitis B, as well as TB test results valid within the last two (2) years must be submitted before the start date. Failure to provide this documentation may result in a delayed start or withdrawal of the job offer.
