Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients.
As a Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative solutions. You will be responsible for performing in-person patient assessments, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care. You'll have the opportunity to travel to designated skilled nursing facilities within your region, fostering direct engagement with patient care and on-site teams.
Responsibilities
In-Person Comprehensive Assessments: Conduct detailed patient evaluations face-to-face for current remotely monitored patients. Gather medical histories and assess current health conditions using both direct interactions and AI-powered insights to spot trends and predict issues.
Facility and/or Home Visits: Travel to designated local skilled nursing facilities or patient homes to support patient care and the facility's care team.
Personalized Care Planning: Develop tailored care plans. Define specific health monitoring parameters, set realistic health goals, and use predictive analytics to refine your clinical decisions.
Accurate Documentation & Compliance: Keep up-to-date, precise records of patient interactions, assessments, and care plans in the EHR system. Ensure all documentation meets regulatory standards and policies.
Peer Training & Support: Train and support fellow providers on in-person workflows, AI tool utilization, and best practices for patient care.
Collaborative Care: Work closely with the Skilled Nursing Facility clinical team, Circadia physicians, Circadia VRNs, and other healthcare professionals.
Team Meetings & Strategy Sessions: Actively participate in multidisciplinary team meetings. Discuss patient care strategies and integrate predictive analytics to drive proactive care.
KPIs
Minimum of 20 patient visits per day or 400 visits per month.
Complete all visit documentation using AI - powered tools within 24 hours of each encounter.
Qualifications
Certified and licensed as a Nurse Practitioner in CA
Medicare participation and ability to have company bill for services on your behalf
Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
2+ years of Nurse Practitioner Experience working with complex adult populations
Ability to practice independently with little or no supervision
Current CA Driver’s License
Comfort using technology such as Google Suite, multiple EMRs, Slack, AI transcription tools
Comfortable in a dynamic and always evolving start up environment
English required, Spanish preferred
